In the general practitioner model, you always consult your general practitioner or a group practice first. Your treatment is coordinated there. In the traditional model, you are free to choose your doctors and can go straight to specialists.
Yes, that is usually the case. Exceptions apply in emergencies, as well as for obstetric care, gynaecological examinations, visits to ophthalmologists or dental treatment.

In an emergency, you can go straight to a hospital or out-of-hours practice. Let your general practitioner know as soon as possible afterwards.
